To benefit from our services you must have out-of-network benefits through your insurance plan. This means your insurance plan will likely be;

  • PPO

  • POS

Plans that are not covered are:

  • HMOs- such as Kaiser

  • EPO- Private plans that are contracted with in-network providers.

This information can be found on your insurance card [usually in the bottom or top corner of the card].
